We offer two Eldercare plans—Standard and Enhanced—for proactive planning, crisis assistance and caregiver support. Included in these plans are the following services:

  • Fact Finding Consultation with Issues and Options Report. Telephone consultation with a Health Care Manager who creates a summary of issues discovered and customized recommendations.
  • Personal Health Care Manager. Ongoing assignment to a personal Health Care Manager who is a registered nurse available to answer any question and to re-assess the care plan and make new recommendations as a senior’s situation changes.
  • Account Management and Education. An account manager who partners with you to answer questions and provide educational materials about caregiving and the benefit.
  • In-home Assessment with Recommendations Summary. In-home assessment by a registered nurse anywhere in the U.S., resulting in a holistic care plan based on the senior’s current living situation. (Enhanced plan only)
